Jose Cruz, a 59-year-old construction worker from Sunset Park, died on Wednesday, April 12, after falling from an I-beam at 1604 Broadway (near W. 49th Street). As NYC Buildings Commissioner Rick Chandler informed reporters that the fall was “completely preventable” and that Cruz should have had “tie offs with his personal protection equipment, which he was wearing,” the Department of Buildings stapled a stop work order at the site.
Streamline USA LLC is believed to be the general contractor at 1604 Broadway and, in March 2016, was fined $19,200 by the U.S. Occupational Safety and Health Administration (OSHA) after OSHA inspectors responded to a complaint about unsafe job conditions at 1604 Broadway. Streamline is contesting those fines which accompanied seven OSHA violations and has yet to pay $17,746 in OSHA fines from other violations at another NYC job site.
